With the participation of the clients, their family member, and/or their legal representative, an RN Care Manager conducts a comprehensive assessment of the client to identify his/her medical physical, psychological, social,and environmental needs. Once those needs are determined and agreed upon, the Care Manager Team, after taking into consideration "Safety and Security", creates a Plan of Care to assist the client in attaining his/her maximum functional potential, and maintain a safe healthy and enjoyable lifestyle.

As a client's advocate, Shining Star Care Management works closely with physicians home health care providers, home medical equipment providers, elder law attorneys, trust officers, guardians, and ancillary service providers to protect the interest of each client and address their needs in a timely and cost effective manner.
