long-term insurance companies, and can help you navigate the system to better understand your Long Term Care policy and maximize your benefits. Shining Star Home Health is not a registry or a referral service. 

After and in-depth assessment by a Shining St

ar RN and with the a participation of the client and/or their family member, legal representative and physician, a personalized plan of care is created for the caregivers to follow. The Care Plan is revised and amended as the clients needs change.

At Shining Star Care Management, our Professional

Team oversees all aspects of care in a holistic approach with Integrity, Experience, and Personal Attention

to our client's needs.

With the participation of the clients, their family member, and/or their legal representative, an RN Care Manager conducts a comprehensive assessment of the client to identify his/her medical physical, psychological, social,

and environmental needs. Once those needs are determined and agreed upon, the Care Manager Team, after taking into consideration "Safety and Security", creates a Plan of Care to assist the client in attaining his/her maximum functional potential, and maintain a safe healthy and enjoyable lifestyle.

As a client's advocate, Shining Star Care Management works closely with physicians home health care providers, home medical equipment providers, elder law attorneys, trust officers, guardians, and ancillary service providers to protect the interest of each client and address their needs in a timely and cost effective manner.
